  • Indications and Dosage & Administration
  • Contraindications
  • Adverse reactions
  • Precautions

  • Therapy in conditions when the anti-inflammatory and immunosuppressive effects of glucocorticoids are needed:
  • Rheumatoid arthritis, systemic lupus erythematosus, some types of vasculitis, temporal arteritis and periarteritis nodosa, sarcoidosis, bronchial asthma, ulcerative colitis, hemolytic anemia, granulocytopenia and severe allergy conditions including anaphylaxis.
  • Treatment against acute leukemia, lymphoma, cancer of the breast and prostate cancer.

  • Administered orally.
  • The initial adult dosage of prednisolone may range from 5-60 mg daily, depending on the disease being treated, and is usually administered in 2-4 divided doses.
  • Children may be given a prednisolone dosage of 0.14-2 mg/kg daily or 4 - 60 mg/m2 daily, administered in 4 divided doses.

Or as prescribed by physicians.

  • Serious infection, except septic shock or tuberculous meningitis.
  • Known hypersensitivity to prednisolone.
  • Viral, fungal, or tubercular skin infection.
  • Administration of live virus vaccines.

  • Adverse effects occur most commonly when using prednisolone in high doses and long-term therapy.
  • Prednisolone inhibits prostaglandin synthesis and thus suppresses prostaglandin effects on the gastrointestinal tract, that include inhibition of gastric acid secretion and protection of the gastric mucosa. Many adverse effects are related to this action of glucocorticoids.

Common

  • Insomnia, nervousness.
  • Increased appetite, indigestion.
  • Hirsutism.
  • Diabetes mellitus.
  • Arthralgia.
  • Cataracts, glaucoma.
  • Epistaxis.

Uncommon

  • Vertigo, seizures, psychoses, pseudotumor cerebri, headache, mood swings, delirium, hallucinations, euphoria.
  • Edema, hypertension.
  • Acne, skin atrophy, bruising, hyperpigmentation.
  • Cushing of syndrome, pituitary-adrenal axis suppression, growth suppression, glucose intolerance, hypokalemia, alkalosis, amenorrhea, sodium and water retention, hyperglycemia.
  • Peptic ulcer, nausea, vomiting, abdominal distention, ulcerative esophagitis, pancreatitis.
  • Muscle weakness, osteoporosis, fractures.
  • Hypersensitivity reactions.

  • Patients with osteoporosis, newly established anastomoses (intestine, blood vessel), psychosis, gastric ulcer, duodenal ulcer, diabetes, hypertension, cardiac insufficiency and growing individuals (children).
  • Because of the risk of adverse effects, systemic corticosteroids should be used cautiously in the elderly, with the smallest possible dose, and for the shortest possible time.
  • Acute adrenal insufficiency may occur with abrupt withdrawal after long-term therapy or with stress.
  • At high doses, interference with vaccination can occur.
  • Pregnancy: Chronic maternal administration of systemic corticosteroids has been associated with a mild decrease in birth weight in human beings. High systemic doses of glucocorticoids may cause small risk for neonatal adrenal hypoplasia. In general, the use of corticosteroids in pregnant women requires that the anticipated benefits of the drug should be weighed against possible hazards to the mother and the child.
Corticosteroids shall absolutely be used in pregnant women, suffering from asthma, since the asthma is danger for oxygen tension for the foetus. Glucocorticoids before premature birth also help accelerate maturation of child-lungs, preventing respiratory distress syndrome.
  • Lactation; Prednisolone is excreted in breast milk less than 1% of the administered dose and may be clinically insignificant. Caution should be exercised when prednisolone is given to a nursing mother.
  • Effects on ability to drive and use machines: None known.
